Please understand that your dental coverage is a contract between you and your employer and that insurance benefits paid by your carrier typically max out between $1,000-$1,500 per year. There are certain services and policies that can be excluded based on the contract between your dental carrier and your employer based on their plan selections. The care we recommend is always based on achieving the best possible result for your specific dental needs and is not based on what your employer and insurance have dictated in their plan limitations.

IMPORTANT

Patients under 18 must be accompanied by a guardian on their first consultation visit.

Please let us know of any relevant medical conditions or medications before surgery in case they may interfere with treatment.
